Think about it that way :

Machine X ------- A.Router.B ------- Machine Y

With AltQ on Router you can shape the traffic going from X to Y and
vice-versa, independently :
- for streams coming from X to Y, you shape it while it's outgoing on
interface Router.B.
- for streams coming from Y to X, you shape it while it's outgoing on
interface Router.A.
